Suicide Crisis Assessment Nurse (SCAN)
Description | This is a new service in County Meath, open since the beginning of February 2024 and as part of the role out plan it is currently only available to GPs within Navan, with plans to expand this imminently. The role of the SCAN service is to provide assessment and support to GP service users who are: • 18yrs and over (including those aged 65 and over) • Experiencing suicide-related thoughts and/or thoughts of self-harm • Not at immediate risk of suicide • Not currently receiving care from a Community Mental Health Team (CMHT) This service aims to intervene early to those experiencing suicidal thoughts and/or thoughts of self-harm and provide a timely assessment and support. Patients can speak to their GPs about their symptoms and if the GP thinks it is appropriate they can make a referral to the SCAN. If SCAN service is suitable the SCAN will make contact with the person within 1 working day and offered an assessment within 3 working days within the GP surgery. The person meets a Clinical Nurse Specialist and receives a comprehensive assessment and risk formulation. A management plan is developed in collaboration with the person and their pre-existing support system to meet their needs and assist onward referral to next care services where appropriate. If required the person can be provided up to 3 points of contact. |

Description | The Rainbows programme supports children and young people affected by loss due to bereavement, separation or divorce. The service is available in local communities throughout Ireland. The Rainbow service is free of charge and is partly funded by Tusla and the Child and Family Agency. Rainbows is available in schools family resource centres and parish centres.
